Drs. Smith and Anderson are accepting new patients. New patient appointments are available Monday - Friday, 8:00a.m. - 5:00p.m. When calling to schedule your appointment, please notify the staff of any day and time preference. Our staff will make every attempt to accommodate your schedule. For your convenience, all new patient forms are available for download. If you like, please complete these forms prior to your appointment. If your insurance company requires referrals for specialists, our staff will be happy to assist; you may also contact your primary care physician for assistance obtaining your referral.
